It used to be that, in order to get your medical records, you had to send a self-addressed stamped envelope along with the completed Medical Records Request Form, and a money order for $5 to our offices for processing.
We thought we could do better, and we have. To make things faster and more convenient for our patients, we’ve come up with some changes. You can still order your medical records like you used to, but now we have a convenient online pay option.
With online medical records requests you can order your medical records in a fraction of the time! Simply fill out the form and pay with your credit card. We accept Mastercard, Visa, American Express, and several other major credit cards.
We also have a new, expedited service that will deliver your medical records much faster. For a little bit more money, you can have your medical records express processed and mailed within the next business day. Fast, Hassle free, and ready to go in 2-3 days.
